FOR CONSULTATION WITH DR WONG
Please ask your family physician or walk in physician (in the case which you do not have a family physician) to fax a referral letter to the clinic of your choice. Please see the details on Home Page. Once received, we will contact you with an appointment. If you would like an urgent appointment, please ask your family physician to contact us by phone. At Grand Genesis Health, we also offer same day consultation with a referral, this will be available at first come, first serve basis.
WHAT TO BRING FOR THE CONSULTATION
You will be required to bring a valid healthcard to every appointment. Please bring a list of your current medications and a list of your allergies. Please make sure the dosages and frequency of your medications are included. If you have used any skin care products or medications for your skin condition, we kindly ask you to bring them with you.
INFORMATION ON ARRIVAL
You will be asked to fill in a medical questionnaire to help us find out more about your health and your skin concerns. We also use this to check that all your demographic details are correct on our records. Please take a few minutes to ensure the form is completed in full. We also ask you to review our office policy during this time.
